Enrollment Forms

CPA firms that perform audits, reviews or compilations are required to undergo a peer review within 18 months of their firm creation date, then once every three years to maintain their license to practice public accounting. 

To enroll, click on the appropriate form, complete the form, and e-mail to Practice Programs Manager Bonnie Olivieri at bonnieo@cscpa.org or fax to CSCPA at 860-258-4859.
