Information Required for Scheduling Reviews
This form provides the administering entity (CSCPA) with information about the firm necessary to schedule the firm's review. Firms due for review from January through May will be given the opportunity to accelerate their peer review prior to tax season. Notification will be sent to firms due from July through December after May 1. The notification will include the firm's peer review due date, review number and firm number, all of which must be included on the completed form. The completed form should be submitted to the Connecticut Society of CPAs within 30 days. Firms with October, November and December due dates are encouraged to complete their reviews earlier in the year.
