Information Required for Scheduling Reviews
This form provides the administering entity (CSCPA) with information about the firm necessary to schedule the firm's review. Notification will be sent to firms approximately 9 to 12 months prior to their peer review. The notification will include the firm's peer review due date, review number and firm number, all of which must be included on the completed form. The completed form should be submitted to the Connecticut Society of CPAs within 30 days.

- Complete the fillable online form (pdf file) and submit it electronically. This is a fillable pdf form, so you may type your information right into the form. NOTE: You must print a copy of the form for your records and for the reviewer BEFORE clicking the "submit" button. The completed form cannot be saved. The reset button clears all data from the form fields.
- Print the blank form (pdf file), complete, and mail to the Connecticut Society of CPAs, or fax the completed form to Practice Programs Manager Bonnie Olivieri at 860-258-4859.
